Special Series: Empowering Women in Transplantation:
The journal commissioned a series by a multidisciplinary group of both, female and male transplant leaders led by Ana Rossi and Anjana Pillai, charged in creating a framework to sustain, recruit, retain and empower women in transplantation. Topics of the May issue cover, i) gender inequities for transplantation professionals worldwide with a call for prioritizing gender equity; ii), analyzing and eliminating hierarchies and biases in transplant teams, and iii), institutional strategies to overcoming academic bullying.

Donor ventilation and Lung Transplantation
Authors from Edmonton, Canada, present the first study that examines the impact of donor ventilation duration on lung transplant function. With a lack of evidence up until now, this study provides reassurance to the lung transplant community regarding the use of donors with extended duration ventilation by demonstrating similar lung function compared to recipients of donors with short ventilation duration.

HLA-DQ and Immunogenicity: Not all mismatches are created equal
The topic of “eplet matching” (epitope matching, molecular mismatch load analysis) in transplantation remains both hot and controversial. Work by Anat Tambur and co-workers from Northwestern University in Chicago identifies “molecular mismatches” that are critical for the generation of de-novo Donor-Specific-Antibodies (dnDSA). These data demonstrate that there is a limited number of regions within DQ chains that are critical for the initiation of dnDSA. This report also shows that additional dnDSAs can be generated later in a process of epitope-spreading with many of the “molecular mismatches” counted by the eplet load approach not being relevant for antibody production. This report contributes with critical detail to the ongoing discussion on the value of eplet matching.

Palliative Care in Transplantation
M. Kenaston and coworkers used a large database including heart, liver, lung, and kidney transplants across three Mayo Clinic sites, representing the largest descriptive analysis of palliative care in the solid organ transplant setting to date. The report highlights the engagement of palliative care before, during, and after transplant. Those observations provide baseline evidence on current practice, identify key gaps, and suggest opportunities for future prospective studies to test the impact of earlier referrals.

Welcome from the International Xenotransplantation Association!

The xenotransplantation community is energized by our recent and hugely successful Geneva Congress. The field of clinical xenotransplantation is moving forward with a set of kidney and liver trials, with multiple new applications to regulatory authorities in process. Data from early clinical experiences suggests that more immune pathways are engaged in the host response to xenografts – uncovered following successful genetic alterations which have addressed some of the well-described target antigens of vascular rejection. Observations made in clinical studies demonstrate the importance of cytotoxic NK-cell and macrophage populations, delayed antibody and complement-mediated injuries and thrombotic microangiopathy. Immune pathways are beginning to be revealed using spatial transcriptomics and multi-omics.

Donate Life Egypt

A recent conference in Cairo (Donate Life Egypt) revealed the importance of determining where xenotransplantation fits into society and medicine at large. In societies where deceased organ donation may not be available due to structural or cultural limitations, xenotransplants may ultimately serve to address gaps in organ availability – if affordable in low- and middle-income countries. Each region must develop regulatory and financial frameworks and microbiological surveillance programs for xenotransplants that complement available allotransplantation resources. Harmonization of database elements will facilitate international collaborations. To this end, IXA is supporting work to develop workable standards for labeling of xenogeneic organs for clinical use.

IXA 2027 - Boston

We look forward to planning for the 2027 Congress in Boston where we will share preclinical and clinical data collected since the 2025 Congress. The IXA Board and our Committees will be defining standards for microbiological assays and screening of donor swine, immune pathways in clinical studies, ethical hurdles and sharing of important data as they emerge from our laboratories and clinics. As seen in the illustration, we are now growing special pigs that will be ready for our meeting! I hope all of you will be able to join us as xenotransplantation continues to blossom!

2025 Highlights in Xenotransplantation

1 - Strong Scientific Exchange at IXA 2025

The 2025 International Xenotransplantation Association (IXA) Congress in Geneva brought together over 400 scientists and clinicians from 21 countries. The meeting highlighted cutting-edge research, international collaboration, and strategic planning for the next phase of translational work.

2 - Continued Success in Kidney Xenotransplants

Multiple centers have completed xenotransplants of genetically engineered pig kidneys into living recipients in 2025. Studies of pig xeno-livers are also underway in China and in clinical trials in the Unites States.

3 - Clinical Trials Begin for Gene-Edited Pig Kidneys

In 2025, the first regulated clinical trial testing gene-edited pig kidney transplants in patients with end-stage kidney disease was launched in the United States. This trial will include multiple participants and could expand if early outcomes are positive, signaling a major regulatory milestone toward making xenotransplantation a practical treatment option.

4 - Molecular Insights into Immune Rejection

Molecular Insights into Immune Rejection At the ESOT Congress 2025, researchers presented detailed mapping of the human immune response in pig-to-human kidney transplants, identifying key cellular players and potential therapeutic targets to prevent rejection. This kind of mechanistic science is essential for improving long-term outcomes.

5 - Expanded Regulatory Activity & Industry Trials

Beyond academic centers, biotechnology companies (e.g., United Therapeutics and eGenesis) received regulatory approvals to initiate clinical studies of pig-derived organs, reflecting industry momentum toward scalable xenotransplantation solutions.

6 - Progress in Other Organs

Gene-edited pig lungs were transplanted into a human recipient (brain-dead “decedent” recipients), providing new insights into lung xenotransplantation biology and supporting proof-of-concept in a previously untested organ system.

Xenotransplantation is the official scientific journal of our society and was established over 30 years ago in 1994. It is currently the only journal focusing on this topic. The journal publishes 6 online issues per year with original articles, reviews and commentaries on recent developments. The journal is available on line to our members. We would like to invite all TTS members who have new data of any aspect in the field to consider submitting these data to our journal.

Rising Stars: The Editorial Fellows Series

Meet our newest Editorial Fellow!
Dr. Quan Yao Ho

Dr Ho is Senior Consultant in the Department of Renal Medicine at Singapore General Hospital and Clinical Assistant Professor at Duke-NUS Medical School. He is currently completing a DPhil at the University of Oxford, focusing on the development and application of regulatory T cell therapies.

Dr Ho's research includes investigator-initiated studies evaluating Torque teno virus for immune monitoring in long-term kidney transplant recipients and pharmacogenomics-guided tacrolimus initiation in the Singaporean multi-ethnic population. He has also published widely on his centre’s clinical kidney transplantation experience, including work on allograft biopsy complications, post-transplant infections, and pre-transplant cardiovascular evaluation.

He is actively involved in undergraduate and postgraduate medical education and has held committee appointments within Singapore’s nephrology and transplantation societies. He is motivated by the benefits that kidney transplantation can bring to patients with end-stage kidney disease and is excited about how advances in transplantation sciences can continue to improve patient outcomes.

Fun Fact: Dr. Ho has previously completed several endurance events, including two Ironman triathlons and two ultramarathons. He now enjoys the simpler pace of nature runs and following basketball games.

WTC 2025 Highlights: Infectious Disease
The World Transplant Congress 2025 had many exciting updates in the field of transplant infectious disease. A brief overview of selected studies is presented here, including research on viruses, bacteria, fungi, and vaccines, based on clinical impact, novelty, and applicability. Many other high-quality abstracts were presented that provided important new data about infections in transplant recipients, which have been published in a supplement to The American Journal of Transplantation.
Organ Preservation Trends in the Era of Perfusion Technologies: Promotion of Global Warming or Back to the Ice Age?
Static cold storage (SCS) has long been the standard method for organ preservation owing to its simplicity, cost-effectiveness, and logistical convenience. During SCS, the organ is flushed with a cold preservation solution (typically at 2–4 °C) and subsequently stored in a hypothermic, static environment—commonly on ice—until transplantation. This technique markedly reduces cellular metabolic activity, thereby limiting ischemic injury.

The correlation between histologic lesions and allograft gene expression data is a subject of significant interest. Most analyses include a single for-cause biopsy from each recipient, precluding serial assessment of gene scores.
