Transplantation Journal November Issue

The November issue of the Transplantation offers a broad, most relevant and exciting collection of manuscripts

Overview

  • Robotic Surgery is getting increasingly relevance for donor and transplant procedures. Indications, challenges, advantages and educational aspects are presented in an Expert Insight contribution by Elizabeth Pomfret representing the perspective of a ‘Non-robotic surgeon’.
  • With many organs currently procured but not utilized for transplantation, understanding refusal patterns is of critical importance. Refusal patterns based on patient, center and organ patterns are analyzed based on most recent Organ Procurement and Transplantation Network data, providing a framework to improve organ allocation.
  • Liver re-transplantation remains a challenging indication. Based on the SRTR data a user-friendly online application is introduced using recipient and donor characteristics to predict 1-year survival. This tool may may support the decision to list candidates while guiding donor selection.
  • And....the first report non-US program report on more than 1,000 pancreas transplants coming from Sao Paulo, Brazil.

This is just a small selection of outstanding contributions. Enjoy the November issue of Transplantation.

Engaging Brain Anti-inflammatory Circuits to Improve the Viability of Transplanted Organs
Solid organ transplantation is a life-saving intervention for tens of thousands of patients each year in the United States. A major underlying pathophysiologic process limiting the success of transplantation is inflammation. Since the first transplant >70 y ago, advancements in the fields of surgery and immunosuppression have improved both organ and patient survival. However, inflammation and its damaging effects remain the principal clinical problem limiting enduring organ transplant survival.
Safety and Tolerability of Sotagliflozin Among Kidney Transplant Recipients
Sodium-glucose cotransporter inhibitors (SGLTi) slow chronic kidney disease progression and reduce kidney failure events. Kidney transplant recipients (KTRs) remain at high risk for these outcomes. SGLTi cause an initial and sustained decline in estimated glomerular filtration rate (eGFR) and have a higher risk of urogenital infection, both of which are major concerns for KTRs. We sought to (1) assess the reversibility of eGFR changes and (2) explore safety and tolerability using sotagliflozin, a dual SGLT1/2 inhibitor.

Identification of Surrogate Outcome Biomarkers in Lung Allograft Recipients With Bronchiolitis Obliterans: Implications for Randomized Controlled Trials
We developed a prospective, multicenter “registry” to evaluate the effect of extracorporeal photopheresis for lung transplant recipients who develop chronic lung allograft dysfunction (CLAD) with the bronchiolitis obliterans syndrome (BOS) phenotype (CLAD/BOS) and who are refractory to conventional standard-of-care therapy.
Potential Impact of Extracorporeal Photopheresis on Trained Immunity and Organ Transplant Acceptance
Extracorporeal photopheresis (ECP) is a well-established, safe, and effective immunomodulatory therapy currently used in clinics to decrease T cell–mediated immunity in various disorders, including autoimmune diseases and chronic rejection in organ transplantation. Although the ECP procedure has been shown to induce apoptotic cells that are reintroduced into the patient at the end of the treatment, the precise tolerogenic mechanisms mediated by ECP are not fully understood.

ISN-TTS Fellowship Awardee

The International Society of Nephrology (ISN) and TTS join forces to support the training of young physicians from low resource areas receiving hands-on training in advanced host institutions to acquire state of-the-art knowledge in transplantation. The ultimate goal of such training is to improve the transplantation standards in their home countries upon their return and become leaders in their fields. The training focuses on providing the skills and knowledge specifically required by the home institution. ISN-TTS Fellowship trainings can be long (6-12 months) or short term (3-6 months).

TTS-ISODP-ESOT - The INTEGRITY Project

The consultation will close 31st October, 2025

Help Shape Ethical Standards in Deceased-Donation Research: The INTEGRITY Project – Public Consultation Open until October 31, 2025

The Transplantation Society (TTS) is partnering with the European Society of Transplantation (ESOT) and the International Society for Organ Donation Professionals (ISODP) to develop ethical guidelines for research involving deceased organ and tissue donation. The project aims to build global agreement on how such research should be carried out to improve transplant outcomes and save lives, while respecting ethical principles.

A draft version of the guidelines is now open for public consultation. Healthcare professionals, donor families, transplant recipients, and interested members of the public are all invited to provide input. The guidelines are available in multiple languages to assist engagement from around the world.

MESOT 2025 - 50th Anniversary of Organ Transplantation in Turkey

The 19th Congress of the MESOT has a very special meaning for us as at the same time, we will be celebrating the 50th Anniversary of Organ Transplantation in Turkey. It was on November 3, 1975 that our team performed the first successful living related kidney transplantation in Turkey from a mother to her 12-year-old son at Hacettepe University Hospitals.
At MESOT 2025, TTS expertise is front and center. The Pre-Congress Workshop—led by Prof. Mehmet Haberal with course chairs Marcelo Cantarovich and Andrea Herrera-Gayol—features a distinguished faculty including Jeremy Chapman, John Fung, Annika Tibell, Stefan Tullius, Andreas Tzakis, and Marti Manyalich. Across the main program, TTS leaders shape sessions and committees spanning ethics/education, kidney and liver transplantation, immunology, and registries, underscoring TTS’s impact on science, clinical practice, and governance. The agenda also includes a dedicated DICG (Declaration of Istanbul) ethics session focused on combating organ trafficking and advancing equitable practice.
