Just Released - Transplantation Direct - January Issue
We begin the new year with a thought-provoking issue of Transplantation Direct. Several topics are represented in this issue, including organ donation regarding broadening of donor liver criteria, professional opinions about molecular donor-recipient matching, and the influence of social deprivation. In kidney transplantation, the effects of transplantation on bone density are explored, as well as a surgical reconstruction method to deal with renal artery dissection. We have multiple articles on pancreas and islet transplantation, from medical and surgical management of failed pancreas transplantation, to thrombosis-related graft inflammation, to issues related to islet autotransplantation. Also, an experience of successful living-donor lobar lung transplantation with perioperative BK virus complications is described. Additionally, this issue publishes abstracts for the upcoming 2023 Transplantation Society of Australia and New Zealand Annual meeting. As 2024 kicks off, we hope that you continue to take advantage of Transplantation Direct as an open access resource to the latest developments in transplantation.

Women in Transplantation 2024 Research Grants
WIT is delighted to announce that requests for applications are now open for the 2024-2026 Research Fellowship Grants for Research in Gender and Sex in Transplantation.
This will be the fourth year that we are offering research fellowships in sex and gender in solid organ transplantation. These fellowships will support the next generation of scientists, furthering our understanding of the importance of sex and gender in transplantation. This is a growing field of interest and the more we learn the closer we are to applying these findings to clinical care to benefit patients and donors.

The Molecular Phenotype of Kidney Transplants: Insights From the MMDx Project
This review outlines the molecular disease states in kidney transplant biopsies as documented in the development of the Molecular Microscope Diagnostic System (MMDx). These states include T cell–mediated rejection (TCMR), antibody-mediated rejection (AMR), recent parenchymal injury, and irreversible atrophy-fibrosis. The MMDx project, initiated through a Genome Canada grant, is a collaboration involving many centers.
Innate Allorecognition in Transplantation: Ancient Mechanisms With Modern Impact
Through the effective targeting of the adaptive immune system, solid organ transplantation became a life-saving therapy for organ failure. However, beyond 1 y of transplantation, there is little improvement in transplant outcomes. The adaptive immune response requires the activation of the innate immune system. There are no modalities for the specific targeting of the innate immune system involvement in transplant rejection.

Therapeutic Donor Kidney Transplant Outcomes: Comparing Early US Experiences Using Optimal Matching
Therapeutic donors (TDs) are individuals who undergo organ removal for medical treatment with no replacement organ, and the organ is then transplanted into another person. Transplant centers in the United States have started using TDs for kidney transplantation (KT). TD-KT recipient outcomes may be inferior to those of non-TD-living-donor (non-TD-LD)-KT or deceased-donor (DD)-KT because of the conditions that led to nephrectomy; however, these outcomes have not been sufficiently evaluated.
A Single-center Exploration of Attitudes to Deceased Organ Donation Over Time Among Healthcare Staff in Intensive Care
Changes to deceased organ donation in the United Kingdom, including establishment of the specialist nurse for organ donation (SNOD) role, have resulted in increased numbers of donations. Have increasing numbers of donations altered attitudes among intensive care unit (ICU) healthcare professionals (ICU staff) to organ donation over time?
